Can a Knocked-Out Tooth Be Saved With Endodontic Treatment?

Dentist examining a patient after a dental trauma injuryA knocked-out tooth is one of the more frightening dental emergencies a person can face. The moment it happens, most people assume the tooth is gone for good, and the shock of the injury can make it hard to think clearly about what to do next. The good news is that a knocked-out, or avulsed, tooth can often be saved when the right steps are taken quickly, and the patient receives prompt endodontic care.

At Burien Endodontics, we treat traumatic dental injuries with a calm, efficient approach designed to give your natural tooth the best possible chance of survival. Dr. Matthew Tomala brings a military background and advanced endodontic training from the University of Washington to every trauma case, combining precise technique with a genuinely comforting bedside manner for patients who are often in pain and understandably anxious.

What Happens When a Tooth Is Knocked Out?

When a tooth is completely displaced from its socket, the periodontal ligament fibers that hold it in place are torn, and the blood supply to the pulp is severed. This type of injury, known as avulsion, most often happens during sports collisions, falls, or car accidents. Because the tooth’s root surface begins to dry out and lose viable cells almost immediately after being knocked out, every minute outside the mouth matters for the tooth’s long-term survival.

This is why we ask patients to act fast rather than wait to assess the situation at home. If you can find the tooth, handle it only by the crown, never the root, and try to reinsert it into the socket right away. If that is not possible, keeping the tooth in milk or a tooth preservation solution on the way to our office gives our team a much better chance of success once you arrive.

How Timing Impacts Your Knocked-Out Tooth Treatment

The single biggest factor in whether a knocked-out tooth can be saved is how quickly it is replanted and evaluated. According to the American Association of Endodontists, timely treatment following recommended trauma guidelines can significantly improve the odds of long-term retention, even though no treatment can guarantee permanent success. Prompt care gives the periodontal ligament the best chance to reattach and heal properly around the root.

Once you reach our office, Dr. Tomala performs a thorough evaluation using 100% digital patient records, diagnostic imaging, and a CBCT scan to assess the root structure, surrounding bone, and overall health of the tooth. This detailed picture allows us to determine the safest path forward, whether that means monitoring the healing process, beginning root canal therapy, or stabilizing the tooth with a splint while the ligament reattaches.

What Endodontic Treatment May Involve

Most avulsed teeth that are successfully replanted will eventually require root canal therapy because the nerve tissue inside the tooth cannot recover once the blood supply is cut off. We generally begin this treatment within one to two weeks of replantation, as the pulp tissue will otherwise break down and can lead to infection or resorption of the root. Our goal throughout is always to preserve your natural tooth rather than move toward extraction and replacement.

During this process, we may also use our Nd:YAG laser to support disinfection, mitigate pain, and promote healing in the traumatized tissue as the tooth recovers. The laser’s targeted energy reduces inflammation and calms the injured periodontal ligament and pulp tissue more effectively than conventional approaches alone, which can speed recovery and improve comfort in the days after the injury. In some trauma cases, this early laser intervention can calm the injured tissue enough that root canal therapy becomes unnecessary altogether, allowing the tooth to heal and stabilize on its own. A few signs that a knocked-out tooth needs urgent evaluation include the following:

  • Visible discoloration developing in the days after the injury
  • Ongoing sensitivity to hot or cold that does not fade
  • Swelling or tenderness in the gum near the injured tooth
  • A tooth that feels loose or sits differently against the bite

If you notice any of these signs, we encourage you to reach out right away rather than waiting to see if the symptoms resolve on their own. Our same-day emergency care is designed to get you evaluated quickly so any developing complications can be addressed before they threaten the tooth further.

What to Expect at Your Visit

We know that walking into our office after a dental injury can feel overwhelming, so we focus on making the visit as efficient and comfortable as possible. From your first moments in the chair, our team will explain each step, answer your questions, and keep you informed about what we are seeing and what it means for your tooth.

Our advanced technology, including CBCT imaging and surgical microscopes, allows us to evaluate root fractures, bone involvement, and pulp health with a level of detail that traditional X-rays cannot match. We also review your treatment options with you directly so you understand what we recommend and why before any procedure begins.

If the tooth cannot ultimately be saved through retreatment or standard root canal therapy, we will discuss the outcome honestly and help you understand the next steps to restore your bite. Our priority is always to give your natural tooth the best opportunity to heal before considering any other option.
